All students who compete in athletics need to have a physical at minimum once every 3 years.  However, Interim Physical forms, Consent forms, and Concussion forms need to be filled out every year.

• Technology Policies

All Parents/Guardians of students in grades 6-12 are required to read and sign the computer handbook annually.  Students and Parents/Guardians of 6th grade students who plan to attend the Kadoka Middle School and 9th grade students who plan to attend Kadoka Area High School are required to attend a technology policy handbook meeting in August.  More information about this meeting will be available around the first of August each year.

High School Students must submit a Parent/Guardian signed Open Campus form before they will be eligible to leave campus during the school day.  Please find the appropriate forms below:
